Timely colonoscopy after an abnormal fecal immunochemical test (FIT) is crucial for colorectal cancer (CRC) screening but remains low, particularly in safety-net settings. Our study implements a multi-level intervention across 12 primary care clinics, combining a clinic-level "best practices bundle" to improve workflow and a patient-level intervention using text and call reminders. We will evaluate the impact of these interventions on colonoscopy completion rates within 180 days, as well as secondary outcomes like referral rates and bowel preparation quality. Findings will inform strategies to enhance CRC screening follow-up in underserved populations.
